Terms and Conditions Certification

The well-being of guests, talent, and staff is the Library’s top priority. Please review the following requirements and acknowledge your compliance below:

HEALTH PROTOCOLS
I am fully vaccinated for COVID-19.  If not vaccinated for COVID-19, I will show documentation of medical or religious exemptions from the COVID-19 vaccine.  You may direct questions to the ADA coordinator by emailing ADA@loc.gov.

Regardless of vaccination status, I will take a PCR test for COVID-19 within 48 hours of the event or a rapid test on the day of the event and will present negative test results upon arrival.  Digital or printed documentation must show: full name, date of birth, type of test received, test date, location of testing site, and negative test results for COVID-19.  Alternatively, guests may come to the Library on March 8 between 5:30 – 6:30 pm to take a rapid test before the event.  One test will cover all events March 8-9.

I will wear an N95, KN95 or KF94 mask over my nose and mouth while at the event except when eating or drinking.  Alternately, I will wear a surgical mask with a cloth mask over it.  Masks will be available to anyone who needs one.  Anyone with a medical reason that prevents wearing a mask must obtain authorization from the ADA coordinator by emailing ADA@loc.gov at least five business days prior to the event.

I will not come to the event if I am feeling unwell, have a fever, or have been exposed in the last week to someone with COVID-19, or similar symptoms.  Anyone who has been diagnosed with COVID-19 in the last 10 days should not attend.

I acknowledge that the ongoing COVID-19 pandemic may prevent the event from taking place as planned and the health protocols may change as required by DC law and based on conditions at the time of the event.
